client agreement form:
As with any business, we want our clients to be fully informed to alleviate any surprises that may result from poor communication! Please read the following carefully as it should explain all the "fine print" details. As always, if you have any questions regarding the client agreement form, please contact us and we will do our best to explain.
on-site organizing:
1. A three-hour minimum is required when scheduling appointments within the Grand Rapids (Michigan) area. Longer hours may be required for
appointments outside the Grand Rapids (Michigan) area.
2. A mileage fee may be charge for clients living outside a 10-mile radius of the consultants home base.
3. For out-of-state travel, arrangements are negotiable and will be assessed
on a per-client basis.
4. Payment (check, cash or gift certificate only) must be received, in full, at
the end of each organizing session. Payment is based on a pre-determined
hourly rate.
5. The client must inform the consultant of any pets, toxic materials or other hazards present at the work site prior to the appointment.
6. The consultant is not responsible for any items which are damaged or items which the client agrees may be discarded, donated, sold or given away.

coaching appointments:
1. A three-hour minimum is required for on-site coaching appointments. A one-hour minimum is required for coaching appointments by phone, e-mail or instant messenger.
2. If travel is required, a mileage fee may be charge for clients living outside a 10-mile radius of the consultants home base.
3. Payment (check, cash or gift certificate only) must be received, in-full, PRIOR to the coaching session. Payment is based on a pre-determined hourly rate.
4. If the coaching appointment is on-site, the client must inform the consultant of any pets, toxic materials or other hazards present at the work site prior to the appointment.

event planning:
1. All potential clients will receive one FREE 30-minute consultation to meet with the event planner and decide if they would like to use our services.
2. A mileage fee may be charge for events outside a 10-mile radius of the consultants home base.
3. For out-of-state travel, arrangements are negotiable and will be assessed on a per-client basis.
4. Payment: a down-payment (check, cash or gift certificate only) will be collected at the beginning of the event planning process. The remaining balance must be paid, in full, at the conclusion of the event. Payment is assessed on a per-event basis.
5. Cancellations: in the event of a cancellation, the down payment is non-refundable.

moving & relocation services:
1. A three-hour minimum is required when moving or relocating within the Grand Rapids (Michigan) area. Longer hours may be required for relocations outside the Grand Rapids (Michigan) area.
2. A mileage fee may be charge for clients living outside a 10-mile radius of the consultants home base.
3. For out-of-state travel, arrangements are negotiable and will be assessed on a per-client basis.
4. Payment (check, cash or gift certificate only) must be received, in full, at the end of each session. Billing is possible for business relocations only. Payment is based on a pre-determined hourly rate.

workshops & classes:
1. Workshops and classes are open to anyone and will be scheduled at different times throughout each year.
2. All materials will be provided unless otherwise noted.
3. Payment (check, cash or gift certificate only) must be received, in full, PRIOR to attending the workshop. Payment is based on a per-class basis.
4. All participants register and submit payment before the posted deadline to be eligible for the class.
5. All material used for workshop and class purposes is the property of Organization Made SIMPLE and can only be used within the context of our classes. If you want to use our material, please contact us first.
6. Cancellations: participants who cancel within 48-hours of the class will recieve a full refund. Participants who cancel with less than 48-hours notice will not receive a refund.

speaking engagements:
1. There is no minimum or maximum time limit for speaking. We will work with you to pick out a topic and discuss all your options.
2. A mileage fee may be charged for events outside a 10-mile radius from the speakers home base.
3. For out-of-state travel, arrangements are negotiable and will be assessed on a per-event basis.
4. Payment (check, cash or gift certificate only) must be received, in full, at the end of the event. Payment is based on a pre-determined rate.

gift certificates:
1. Gift certificates may be purchased for any amount.
2. Gift certificates may be redeemed for any of our services.
3. Gift certificates that are purchased will never expire.
4. Gift certificates that are donated by Organization Made SIMPLE as part of a fund raiser or other charitable cause will expire four months after the date it is issued.
5. Gift certificates are only valid if they are signed by Andrea Dekker.
6. Gift certificates that are copies, reprinted or duplicated in any form will not be valid.
